Unable to convert old dogmi to new dogmi tokens

@Dogmi @ICPSwap @dfiniitybot i am trying to convert old dogmi tokens of 10% to new dogmi sns token as mentioned. I followed steps and transferred successfully tokens to kw5rk-raaaa-aaaai-qpfoa-cai from step 1. No clue of my tokens were it went. I am gettin below error and shows my tokens 0 sometimes from step 2.

Error below, please kindly help on the proper process, that i can do easy convertion of my old tokens.

please help me with transferred funds to kw5rk-raaaa-aaaai-qpfoa-cai. How can i know the status of it.
